For your protection and ours, LA Carriers, LLC does not accept W‑9 forms submitted electronically from individuals or sole proprietors. If you are an individual (including independent contractors operating under your name) or a sole proprietor, please do not upload, email, or otherwise transmit your completed W‑9 through online forms, portals, or unsecured email.

Why:

• W‑9s contain sensitive personal information (Social Security Numbers, home addresses) that is vulnerable to interception and identity theft when transmitted online.

• Our systems and third‑party platforms are not configured to securely receive or store SSNs for individual/sole proprietor taxpayers.

• State and federal privacy rules require extra safeguards for personally identifiable tax information.

How to provide your W‑9 instead:

• Mail a printed, signed W‑9 to our business address: LA Carriers, LLC, [Company Address]

• Deliver a printed, signed W‑9 in person to our office during business hours

• Contact our accounting department at [phone number] to arrange a secure alternative method

If you are a corporation, LLC taxed as a corporation, partnership, or other entity that does not use an individual’s SSN, you may be permitted to submit a W‑9 electronically. Please confirm eligibility with our accounting department before submitting.
